A Simple Test Form

This uses a HTMLet to present a simple form for the user. It is embedded into this article using a htmlet tag.

Notes

• The htmlet tag must not have any whitespaces eg, carriage returns in it! Otherwise it believes it has a file name terminated with a blank character.
